Forms and surveys

Online forms and surveys

An online form builder that writes every answer straight to the contact record, tags the person and starts the follow-up the moment they press submit. Forms, multi-step surveys and scored quizzes all live on the same record as your pipeline, so no enquiry waits for someone to read an email notification.

The problem

Your forms collect answers that nobody acts on

Most companies already have a form builder. The trouble is what happens after submit. The answers land in an email notification, someone copies them into the CRM when they get a minute, and the person who filled in the form waits. By the time they hear back, they have spoken to someone else.

Surveys go the same way. Feedback is collected, exported to a spreadsheet and read once. Nothing in the business changes because of it. Here, every form, survey and quiz writes to the same record your sales, marketing and service work runs on, so an answer can start the next step on its own.

Answers sit in an inbox

Form notifications are read late, forwarded, and sometimes lost.

Details are re-keyed by hand

Someone copies names and answers into the CRM, with mistakes.

Every enquiry looks the same

A large project and a casual question get the same slow reply.

Survey results go nowhere

Feedback is exported, read once and never linked to a client.

Submit is the first second,

not the last step.

What happens to a lead

Where form enquiries leak today

Count the enquiries your forms already produce and see how many never become a conversation. Use your own figures.

Your pipeline
In your currency.

Based on the figures you entered

Enquiries that never get a reply, per month20
Replied to but never a real conversation, per month32
First-year revenue at stake each month€6,000 to €22,000

This is your own arithmetic on the figures you entered, not a result levelpartner.ai has produced for anyone.

How this is calculated

value of a booked meeting = your close rate × first-year value

low estimate: enquiries with no reply × your conversation rate × your booking rate × value of a meeting

high estimate: the low estimate + enquiries replied to without a conversation × your booking rate × value of a meeting

The low figure counts only enquiries nobody replied to. The high figure adds enquiries that got a reply but never became a conversation. Not every one of them was winnable.

Worked example. 100 enquiries, 80% replied to, 60% of those reach a conversation, 40% of conversations book, 25% of meetings close at 5,000: 20 enquiries get no reply and 32 never reach a conversation, roughly 6,000 to 22,000 of first-year revenue at stake each month.

What it does

A form builder and survey software on the same record as your CRM

Forms, surveys and quizzes are built in one place and every response belongs to a contact, not to a spreadsheet.

A drag and drop form builder

Build enquiry, application and order forms by dragging fields into place: text, dropdowns, checkboxes, dates, file uploads, signatures, monetary amounts, a terms checkbox and a captcha. Any contact or deal custom field can be dropped in, so a custom form builder project becomes an afternoon.

Creating an online form

Conditional logic that qualifies

Rules show or hide fields, show a message, send the visitor to a page or disqualify the lead, based on any answer. Conditions combine with and or or, and run top to bottom. On a booking form a disqualified answer stops the booking before it is made.

Online survey software with branching

Surveys run one question per slide and jump to a different slide depending on the answer, which keeps long questionnaires short for each person. Calculated fields add up scores, quantities or estimates from the answers as the visitor types.

Creating an online survey

Scored quizzes with a results page

Give answers points, total them into a score, a category or a tier, and show a results page with a button to the right calendar. The score is written to the contact, so a workflow can follow up by result.

A website form builder that embeds anywhere

Use a form on a page built on the platform, embed it on your own site with a snippet or share it as a link. A hidden source field records where the enquiry came from, and returning visitors can be remembered so they do not type their details twice.

Building a lead capture form

Every answer starts something

A submission can create or update the contact, add tags, open a deal, assign an owner, send an internal alert and an automatic reply, and start a workflow. The enquiry is answered while the person is still on your page.

One form, as it is set up
  1. Fields name, email, phone, project type, budget
  2. If project type is "new website", ask timeline
  3. On submit create contact, tag "website enquiry"
  4. Then send a reply with a booking link
  5. And open a deal and alert the owner
Building one properly

A form that qualifies, in four decisions

The builder is quick. The thinking is what makes the form earn its place on the page.

  1. Ask only what decides the next step

Name, a way to reply and one or two questions that choose the owner or the offer. Everything else can be asked on the call.

  2. Map each answer to a field

Use contact or deal fields, not loose text, so answers can filter a smart list, set a deal value or appear in a report.

  3. Add the rules

Show follow-up questions only when they apply, disqualify what you cannot serve and send good fits straight to a booking page.

  4. Wire the submit

Tag the contact, open the deal, alert the owner and send a reply with a booking link, all from the same submission.

Before and after

A standalone form tool against forms on the platform

Separate form tool

  • Answers arrive as an email notification
  • Someone copies them into the CRM
  • Follow-up starts when someone has time
  • Survey results live in a spreadsheet

Forms on the platform

  • Answers write to the contact record
  • No copying, no second login
  • A reply goes out the moment they submit
  • Every response is linked to a person

The form itself looks much the same. What changes is that nothing waits for a person to move the answers.

A form is where an enquiry starts, not where it ends.

FAQ

Questions buyers ask

What is the best online form builder for a growing company?

The one whose answers go straight into the system your team already works in. A good-looking form that only sends an email still leaves someone to copy the details across. Here, every form writes to the contact record and can start follow-up on its own.

Is there a separate charge for forms and surveys?

No. Forms, surveys and quizzes are part of the platform licence. Messages sent after a submission, such as SMS and email, are billed as used.

Can I use the forms on my existing website?

Yes. Each form can be embedded on any site with a short snippet, used on a page built on the platform, or shared as a link. If you would rather keep a form you already have, a tracking script can capture standard HTML form submissions from your site into the CRM; forms inside an iframe or a third-party widget are not captured that way.

Can a survey route people differently based on their answers?

Yes. Questions can branch to different slides, answers can carry a score, and the score or a specific answer can decide which workflow starts, which tag is added and who owns the contact.

How long should an enquiry form be?

As short as the next step allows. Ask for a way to reply and the one or two answers that decide who calls back and what they offer. If you need a full fact-find, send it after the first conversation, when the person has a reason to finish it.

Can we take a payment or a signature on a form?

Yes. A payment element takes a card through your connected payment provider, and a signature field captures consent or agreement on the form itself. For a full contract, use a document sent from the deal instead.

Should we use it for anonymous staff surveys?

Not if anonymity matters. Every response here belongs to a contact, which is the point for client feedback but the wrong design for an anonymous engagement survey. Use it for named pulse checks and client surveys, and a dedicated tool where people must not be identifiable.

Can we move our existing forms across?

Yes. Existing forms are rebuilt during onboarding, with the same fields and the follow-up you want behind them. Past responses can be imported to the contact records they belong to.
